35,M with severe tear trough hollowness - What is the best treatment approach? (Photos)
Hello Doctors, I am a 35-year-old male with significant under-eye hollowness and dark circles. Part of the darkness appears to be genetic, as my parents and sister have similar under-eyes, but I also have a very deep tear trough that creates substantial shadowing. I recently had a consultation and was told that my case may be more complex than a typical under-eye bag patient because the primary issue appears to be volume deficiency rather than excess fat. In the first photo, I intentionally squinted so the anatomy would be easier to see. There appears to be a small amount of fat beneath the lower eyelid, but not a significant under-eye bag. Based on the attached photos, what treatment approach would you consider most appropriate for my anatomy? -Lower blepharoplasty with fat repositioning? -Fat grafting? -Tear trough implant? -A combination of procedures? -Something else entirely? My main concerns are the deep hollowness, shadowing, and overall tired appearance. I would greatly appreciate your professional opinions on what you believe would provide the most natural and effective result.
