Dear Tylermg, Thank you for posting your photo. From the photo, although not as useful as a live clinical examination, it appears you have a very superficial line or wrinkle immediately under your eyelash and then a deeper groove towards your inner aspect of your eye. The wrinkle under the eyelashes is usually from a hereditary degree of excess skin, especially when you smile, which tends to crease in the locations your photos indicate. The second, and deeper, groove towards the inside is usually hereditary at your age and is called a tear trough or an area of the eyelid that is somewhat sunken. This generally runs in the family and can have a hereditary component. In general, healthy exercise, dieting and sleep will minimize any effects of lifestyle. However, these are likely anatomical in causality and may require either injection therapy to build up volume, which can either be from your own fa or from off-the-shelf tissue fillers and, for the lower lid skin excess, you could try an injectable soft tissue filler placed in the right level, perhaps a small amount of Botox®. However, at your young age, these two temporary treatments will seem frustrating as they will require recurrent injections. Sometimes, for hereditary peri-ocular problems, small excisional-type surgery under local anaesthesia can be restorative, as this is not aging but, rather, hereditary in nature. Seek the advice of one or two certified plastic surgeons with experience in this area or learn to live with some imperfections which, of course, is the least expensive and the safest.
