You are likely confusing Botox, which is used to treat dynamic (motion-related) wrinkles, with fillers injected in certain locations. Botox, whose consistency is like that of plain physiologic salt water, has therefore no chance of clogging blood vessels and hence NO risk for the development of necrosis. I have personally been treating patients for aesthetic purposes since 1991 and have never had a case of necrosis or seen one reported with Botox. Hopes this allays your concerns.
Botox is neurotoxin that inhibits muscle contractions. The side effects of Botox can include but are not limited to lazy eye, swelling, bruising, muscle weakness, etc. Necrosis, cell death, however, is not a common side effect of Botox.

I have never seen necrosis after Botox injection. Side effects can include unwanted drooping of eyelids for example if injected initial the muscles involved in lis opening, facial nerve weakness in the distribution of the muscles of the face inbervated by the 7th nerve. The expectation is return of muscular function after the effects of Botox are gone, generally about 6 months after injection.
