I’m interested in getting a silicon cheek implant as well as a intraorbital rim fat graft or implant, do you think that these would help to add projection to my mid face and are intraorbital fat grafts permanent?

based on this single photo it looks like you have mild to moderate cheek hypoplasia (malar hypoplasia, small cheeks). This deformity can be successfully corrected with combination of cheek implants and fat grafts. In person consultation with a plastic surgeon experienced in facial implants andd fat grafting is recommended. Good luck.

There are a number of options for mid-face augmentation other than facial fillers. The main cheek implant options are malar implants that augement the cheekbones. Sub malar mid-face implants fill our hollowness in the front of the face. Combined malaria-submalar implants does both. There are also orbital rim implants. Lastly, implants can be custom made from a 3-D CT scan for a precise match to your needs.

The mid- face, the cheeks, cheek implants and the maxilla are all three-dimensional structures, and an augmentation occurs in all three dimensions. A full set of pictures and an in-person examination going to be required to make a determination about how best to proceed. The only assured method of midface augmentation, that provides permanent volume and its distribution in the desired areas, is with a custom design midface implant made from the patient's 3D CT scan. Fat grafting does not have a proven track record of consistency in young leaner patients of long-term retention. It is perfectly acceptable to try it first as it has few adverse side effects other than its unpredictibility of retention.

The most ideal approach is to enhance the lack of 'bone' contours rather than mask it with fat 'soft tissue'. Your lack of projection in the area you mention would be improved (after review) with implants to the 'cheekbone/infraorbital rim'. Especially at a young age, one wants to define support and not have a feminine 'softness'.
