Most importantly here is the decision between a mini tummy tuck and floating the umbilicus vs. a full tummy tuck and transposing or moving the umbilicus. It would certainly be easier to revise the scar with the full/transposing option. You are very well educated in your understanding that by floating the umbilicus, the blood supply can be threatened and a scar revision may best be delayed. Additionally, when a mini tummy tuck is performed, the umbilicus will move downward 1-2cm. This may stretch or otherwise change the scar. I think that if you do in fact opt for the mini tummy tuck then it would be wise to wait until you are fully healed from that procedure and then do the scar revision under a local anesthetic in an office-based procedure. Floating the umbilicus (releasing the stalk) mitigates the need for an incision around the umbilicus. Depending on the degree of revision necessary, a skin incision may compromise the blood supply to the umbilicus leading to healing difficulties or in a worst case scenario necrosis.It is always safer to stage procedures when vascularity is a concern.As always, discuss your concerns with a board certified plastic surgeon (ABPS).

To revise the skin above the belly button, the float cannot be done because of the risk of cutting off too much circulation to your belly button. You will have to choose one or the other.
