My sister was told that a full wide breast is needed from at a consult. She is narrower that me in the chest width and from what she is telling me, she is not getting the best advice. I just want to understand the purpose of each

In order to tighten the skin envelope, skin excision is necessary; this results in the presence of scars. The lower the patient's breasts on her chest wall, the greater the degree of lifting necessary (and usually the greater scarring present). I am not familiar with the terminology “wide breast lift”. Sometimes, the presence of scars is a “dealbreaker”; patients would prefer to leave their breasts unchanged than to have scars. At other times, patients prefer to have the improvement in breast position, shape, and (possibly) size and are willing to accept the trade-off of scars. Your sister may be best served by meeting with additional well experienced board-certified plastic surgeons in person to discuss her goals in front of a full-length mirror. But I have found that the vertical breast lift described most recently by Dr. Hall-Findlay can be slightly varied and used effectively for almost all patients. I use this technique for both breast lifts and breast reductions and the results are very good, in my opinion. This type of lift requires only a vertical scar and one around the areola and does not involve the use of a horizontal scar along the breast crease. Even without seeing a photo, I can almost be certain that this type of breast lift would be effective.

There different techniques to do a breast lift, based on the size of the breasts, the surgeon's training in doing different techniques based on the patient's anatomy and needs. Most surgeons in this country still use the wise pattern or anchor scar technique (full wide breast lift?). I personally prefer the vertical or lollipop technique which results in a vertical scar and a scar around the areola. I've been doing this approach to breast reduction/lifts for the past 14 years. It does have the best long term shape to the breast with a short scar. See a board certified plastic surgeon for another opinion or have your surgeon explain his/her terminology so you can understand it. After all, it is your body, and need to be able to form an informed decision. Good luck.
