Hi Hhh1225, In my opinion, the only reliable way to achieve upper pole fullness is to use an implant in addition to the lift. Lift alone will not change the upper pole at all. An alternative is to undergo fat transfer to the upper pole. This definitely works but may require multiple sessions for adequate long term results.Sincerely,Ary Krau MD FACS

If you do want a lift, I think a vertical lift will produce a nice improvement with less scar than an anchor lift.Find a board certified plastic surgeon who performs hundreds of breast lifts each year. Many breast lift patients have received beautiful results without implants. Like you, they are happy with their breast size and were not in need of an implant to obtain the lift and upper pole fullness they desired. While your photo is helpful, it's difficult to know exactly what is needed without an examination. A consultation with a board certified plastic surgeon will provide you with the specific answers you are looking for. In response to your question, yes, it is certainly possible to obtain great breast lift results without implants.
You certainly can improve the shape of your breasts and elevate the position of your nipple and areolae with a breast lift. A breast implant would only be needed if you are trying to add volume to the upper portion (upper pole) of your breasts. A vertical breast lift procedure can add some minimal volume to the upper pole, but if you are looking to have a very full upper portion to the breast, you will be needing an implant. Find a good local surgeon and look at before and after photos of patients that have only had the breast lift surgery. This is the best way to see what is possible with breast lift alone.
A breast lift will lift the breast tissue and will give you little bit of fullness but not the exaggerated fullness that typically comes with an implant. It really depends on your desire for additional fullness in the upper pole. You have a lot of your natural breast tissue so it may be that you are happy after the lift.

In the short term you will have more upper pole fullness with a breast lift alone but this is short lived. The only reliable way to achieve upper pole fullness is with an implant at this time. Even fat transfer will not reliably achieve this. Take a close look at before and after pictures of patients who have had a lift from the side view. Make sure they are more than one year post surgery and you will see that the upper half of the breast looks the same.This is because the skin which is weak cannot support the weight of the breast. For the same reason that you need a lift, the skin of the breast will not support itself.
A breast lift alone would elevate your nipple position and give you some extra fullness at top. If you are interested in having significant added upper fullness, this would best be achieved with implants in addition to the lift.
