Since you are already 4 years post-op, and swelling that occurs acutely should raise your suspicion for an infectious cause. The best course of action would be to follow up with your surgeon, and the second best would be to receive evaluation from a qualified physician to evaluate and treat you as necessary.

I advise that you see a local rhinoplasty surgeon as soon as possible, and likely start a course of antibiotics. Silicone implants carry a risk of becoming infected and in some cases require removal. Let's hope that's not the case, but this needs to be evaluated and monitored in person.
