I recommend an eccentric lip lift. This does 4 things: shortens the distance between the nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. This takes 1 hour to perform under local anesthesia and costs $4,000.

you have two ways of going about this. you could see if your surgeon is willing to do more excision and/or explain why she wants to do 5mm (it may be related to some things you're not aware of anatomically). it is definitely better to err toward more conservative with lip lifts because you can't return back lip but you can always excise more if needed (of course the best case outcome is always to get it right the first time, but one has to account for weird healing). an alternative route is to go to someone else and see if she would do the same vs. more. if that philtral distance on you is really 1in based on the photo and assuming you have literally no dental show, then you probably do need a little more excised but it's hard to know without seeing how much phitlral excess you have.

That's unwise, 5 mm is a large, really large or better said aggressive amount of indirect lip lift; better stay cautions, this is a de facto irreversible maneuver leading to very explicit and obscene scars, eventually might not pay off for the potential benefit you are describing; give it second thoughts.
