I appreciate your question. The size of implant best for you is dictated by your chest wall measurements. Once we determine that we can choose the profile based on what you want or need to achieve. If you are seeking a natural look, then the diameter of the implant should be equal to or, more ideally, smaller than the width of your breast. The breast width is a measurement of how wide your breast is at the base, which should be measured at the level of the nipple. Choosing an implant that is smaller in diameter than your breast width will avoid the "side breast" fullness that is often associated with a more artificial appearance. Other than that, you should choose the implant based on volume, not on the dimensions of the implant. You should choose a board certified plastic surgeon that you trust to help guide you in this decision. Silicone will give you a fullness at the top (upper pole fullness). Silicone implants come pre-filled with a silicone gel and are the softest implant available. They feel more natural, which makes them a good option for women with less natural breast tissue; but they require a larger incision. It may be more difficult to realize if this type of implant has ruptured, so it is important to monitor them with annual follow-up visits. Additionally, because this implant contains a more liquid silicone (less cross-linked), if this implant should rupture, it will leak only into the scar capsule formed around the implant but may cause some discomfort or implant distortion. Anatomic gummy bear implants might be a good choice to give you volume. These highly-sought-after, anatomic implants offer a look that more closely resembles the natural silhouette of a breast, and, therefore, are a very attractive option for individuals seeking a natural-looking, aesthetic primary breast augmentation. Additionally, these implants are an especially excellent option for patients undergoing restorative or corrective breast surgery because they provide more stability, shape, and reduced incidence of capsular contracture. Compared to other types of silicone gel implants, the silicone in the cohesive gel implant is more cross-linked; therefore, should the implant shell “rupture,” it maintains its shape and silicone does not leak. During your breast augmentation consultation, you should feel the different types of implants available, and try on various implant sizers in front of a mirror to help you to get an idea of how you will look following the surgery. You should also bring pictures of the look you would like to achieve, as well as a favorite top to wear when trying on implant sizers. There are likely hundreds of different implants. They come in different shapes, sizes, firmness and textures. The height, width, and projection are all modifiable. It is really great to be able to select from some a large number of choices to get the perfect implant for patients.The more important step is to decide what the perfect implant is for you. I spend a lot of time going over a number of different strategies to come to that decision with patients. It takes a bit of work of my patient's part and a bit more time in the operating room. I usually see my patient an extra visit to be as accurate as possible. During the surgery I bring a trial sizer of each of the possible implant choices. Although several may look good, there is usually one perfect implant. The extra time during surgery to find it is well worth it. To date I have never had to change an implant as we have been able to select them well. Make sure that your surgeon takes the time to help you pick the perfect implant. A redo surgery would be a shame if it could have been avoided with a bit more effort up front.
Far more important than the technique is the skill and experience of your plastic surgeon. There are round and shaped "gummy bear" implants. These implants come in many different profiles and sizes. I recommend you undergo a consult to determine the best size for your frame.
Both Allergan and Mentor make highly-cohesive silicone gel implants, and there are a great number of sizes and shapes, with both round and anatomic shaped implants, and differing degrees of projection available. Because of this your best option is to consult with a board-certified Plastic Surgeon who performs a large number of breast implant procedures, and has the ability to advise you based on your desires and existing anatomy. The subtleties of a well performed breast enhancement require years of experience.
All implants come in a wide range of sizes and dimensions typically from 125cc to 800cc. As a surgeon, I chose the dimensions I need to achieve the visual result a patient desires given the specifics of her anatomy. While we end up with a "volume size", the decision on which implant to choose is not based on volume since the same volume can look radically different in a different profile.The specific size information is readily available on line.
The term gummy bear implant is somewhat confusing and poorly defined since for some patients it refers to all cohesive gel implants and for others it refers to more firmly cohesive anatomic shaped implants only. My preference is strongly for the use of round cohesive smooth silicone gel implants and I make the selection based on the availability of a variety of implants and sizers during the surgical procedure when actual results from a specific implant and volume can be determined visually.
