Answer: Subcision Useful For Raising Depressed Traumatic Scars; Microneedling Helpful For Blending Them With The Surrounding Skin First, let me say how sorry I am that you are so distressed. Although there is currently no form of treatment that can completely eliminate a fully mature (over three months old) scar from any cause, including, as in this case, trauma, we do have some simple and relatively inexpensive methods for improving their appearance significantly. Since the scar is depressed (indented, atrophic), it needs to be elevated so that it becomes more flush with surrounding skin and less able to stand out as a result shadowing in the crater with tangential (side) lighting. Subcision, with or without the adjunctive use of a biostimulatory filler would be a simple, quick and relatively inexpensive approach to achieving this. Once this is done, then microneedling, another quick, simple and relatively inexpensive approach, may be used to blend any residual textural and color irregularities. For more information on both of these techniques, you can check out the archives of RealSelf.com. In doing your vetting, be wary of medspa places, as well as others hawking so-called "next-big-thing"--to-come-along, "one size fits all," relatively expensive light and energy-based devices, such as fraxels, whose hard science to support their uses lags far behind the heavy marketing and advertising they get these days. Make sure that you consult with a board certified aesthetic physician. Best of luck.

Answer: Scar revision Hello and thank you for your question. The best advice you can receive is from an in-person consultation. Based on your photographs, you are a candidate for a scar revision with surgical excision of your scar and multi-layered closure. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon
