I have a large nose with a bump, and I would like not only the bump taken out, but also the size of the root of my nose shaved down so that I have a more defined transition from forehead to nose, is this difficult to do from the surgeons viewpoint or even possible?
Answer: Is it difficult for a surgeon to reduce radix/root size of my nose during rhinoplasty? The areas that concern you can be realistically improved to produce a more refined feminine appearing nose by undergoing a natural appearing artistically performed rhinoplasty. Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you what to do based on two dimensional photos without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best surgery. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else. I would suggest that you find a surgeon certified by the American Board of American Plastic Surgery and one who is ideally a member of The American Society for Aesthetic Plastic Surgery (ASAPS) ) or a facial plastic surgeon (otolaryngologist) that you trust and are comfortable with. Based on your photos, I recommend you have a consultation with a rhinoplasty specialist who does computer imaging. With your hump removed, I'm not sure a radix reduction would be necessary. Answer: Rhinoplasty candidate for dorsal hump and radix reduction A closed rhinoplasty approach can accomplish deepening the radix and shaving down the dorsal hump with all of the incisions placed on the inside of the nose. The high radix is simply an extension of the bony dorsal hump which is removed during the rhinoplasty procedure. Osteotomies of the nasal bones are also required. Anticipate a cast placed across the bridge of the nose for one week along with visible bruising and swelling for 2 weeks. Shaving down the radix is very commonly done by an experienced rhinoplasty surgeon. Rhinoplasty is one of the most difficult procedures to perform correctly in the entire field cosmetic surgery, so choose your surgeon very wisely based on extensive experience. Computer imaging performed at the time of the office setting would be helpful to get an idea of what can be accomplished with the procedure. I believe you have been told about the difficulty in radix reduction because for patients with a very high root of the nose - where the forehead blends in with the nose without a step off - reduction might not be possible. I do not think this is your case.I would recommend a photo simulation for you so you can better define your nasal shape goals with your surgeon and determine if your goals are attainable. good luck

Answer: Radix reduction It is not difficult, and it can be done at the same time in your rhinoplasty. I do thin that some minor reduction of the radix will be needed but not too much.Go for it, you can achieve a really good improvement with a rhinoplasty.Best of luck,
