Dear eejaayy,bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions which is why I try not to use the inframammary approach to any breast surgery. Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies. If you are concerned, please check it with your plastic surgeon. Daniel Barrett, MD, MHA, MS

Dear e,   Thanks for submitting your before and after pictures. From observing your pictures, I agree with you that your left breasts is bottoming out. Also,  the cleavage on that side is wider as well.     At this early stage , there is still a chance to self correct , by wearing a bra with a wire on the left side ,  trying to elevate the crease and doing  medial and superior implant displacement massage, in order to improve on the cleavage.  I would give it a trial for 2-3 months and if no satisfactory improvement occurs, you might have to have revision surgery that includes inferior capsulorrhaphy (internal bra) and medial capsulotomy (scar release) , to give you a nice cleavage.                      Best of luck,                                          Dr Widder
