Hello doctors! Last summer I underwent a double jaw surgery and my face has changed. Also my nose has changed. I didn't really like it before but now I don't like it at all ... It became wider ... What do you recommend me to do? I don't want a big change. It was fine like I had it before. When do you think I can have my rhinoplasty? Or I still have to wait since I did the double jaw surgery in July. Thank you very much for your help. best regards!
Answer: Flat nose after upper and lower jaw surgery Surgery for class III malocclusion where one or more jaws are moved by sagittal split, and Lefort I, can cause nasal flattening as mentioned by experts in this thread. The nose is a fixed structure and sometimes the nose gets flattened against a forward moving upper jaw. This is commonly treated by a followup Rhinoplasty. You have a pretty nose yet some narrowing can always be done. The change in your face has been stunning, yet sometimes the nose can be affected in thisway. Check with your maxillofacial surgeon and see if the repair is stable and whether it is a good time to consider nasal work. If your maxillofacial surgery is plastic and reconstructive trained with a fellowship in dual jaw surgery, he or she would be the best person to perform the Rhinoplasty. If not, then select a board certified facial plastic surgeon or plastic surgeon to advised you.

Answer: Rhinoplasty for wide-flat nose Since it has been over 6 months since your double jaw surgery, it is acceptable to undergo a rhinoplasty at this point. A rhinoplasty procedure can address narrowing the existing bridge line with osteotomies placed in nasal bones, a tip rhinoplasty to narrow the tip, and an alar plasty can narrow wide nostrils. The nose will still widen somewhat with smiling due to the attachment of the facial smile muscles to the base of the nose. In addition, the bridge line can be built up with a cartilage graft which also improves the height of the nose and gives the illusion of less width.

At the time of your rhinoplasty, the base of the nose can be narrowed with an internal suture. This can be corrected (restored) at the time of your rhinoplasty through either an internal alar cinch procedure or external nostril narrowing. Now that you are six months after your facial osteotomies, you can proceed with your rhinoplasty at any time.
