Hi! I had my 1st tip plasty a year ago and started noticing this bump (believe it's a bossa) on my nose tip a couple of months ago. It's not until now it's really bothering me. I have very thin skin and even before surgery my cartilage was a bit visible. What can be done to fix it? I really want to avoid a revision. Could microneedling help thicken the skin and camouflage it? What would happen if I microneedle over the cartilage? Could it improve it ? Thank you

Answer: Tip deformity Hello,The issue is due mainly to the shape of the tip cartilages. It is also possible that there is a graft called a shield graft in the tip. If so, it seems it might be too big and pronounced for your nose and skin type. Unfortunately microneedling will not help this issue. Although it can promote collagen formation. It does not thicken skin in this way, and certainly not enough to disguise the shape. If nonsurgical is the only option, filler can be used to soften this area, but at the expense of making the tip bigger. The ideal solution would be surgical with the main focus of changing cartilage shape. If it is also determined that the skin needs to be thickened, a temporalis fascia graft can be used. Make sure to consult with surgeons specializing in revision rhinoplasty, even for non-surgical correction. This area has many vessels and healing creates a tighter area, which if not respected during injection can create issues with necrosis. On some occasions it is even necessary to inject in small amounts over a few sessions. Good luck!
