Thanks for sharing your concerns. Treatment of alar pinching requires proper analysis of the causes and treatment of all contributing factors. Alar spreader grafting alone is usually not enough. Based on your pictures, it is possible that alar spreader grafts, alar batten grafts (which go under the outer or lateral sides of the tip cartilages), and tip sutures (to reshape and refine the tip itself) would be beneficial. An open rhinoplasty approach would likely be used. Consult with a plastic surgeon familiar with revision rhinoplasty. Good luck. 

Alar spreader grafts as well as valve spreader grafts for revision rhinoplasty.I have use these for 35 years for revision rhinoplasty to correct pinched tips as well as your valve stenosis to improve both the cosmetic appearance and your airway. You should see a revision 

A pinched tip can be corrected with an alar spreader graft. Breathing issues are another thing.  It depends upon what the etiology of your breathing problem is.  An exam in person is key.

Alar spreader grafts will correct a pinched tip as long as there is adequate tip cartilage remaining after your primary surgery. Otherwise lateral crural grafts may be necessary. Regardless, the appearance and function can be improved. I've seen many similar problems but unfortunately I cannot make the exact diagnosis in this forum.
