In general, for optimal results in treating perioral wrinkles, be they of the so-called "smoker's lines" or divets, the use of both a neuromodulator, such as microdroplets of Botox, and a cohesive, low-viscosity HA filler, can work synergistically. The Botox addresses the underlying muscular activity that gives rise to the lines and divets and the filler addresses the etched in lines in the skin that have resulted from years of daily use of those muscles. Microneedling, on the other hand which I have been professionally performing for well over two decades, is best reserved for treating very superficial, surface irregularities in skin color and texture.
