Sinuses issues post-op. What should be the gold standard treatment in that case?

Hi, I underwent a Septo rhinoplasty a year ago. 3 months post op, I started woke up with constant runny nose, Few months later, Starting having chronic post nasal drip as well. I assumed that there is some sort of sinuses issue. Got a post OP CT scan. What should be the gold standard treatment in that case? I don׳t want to make a one larger opening... Please look at the right side which seems to be damaged.

CAT scan is x-ray technology. Air is black on x-ray. When we look at a CAT scan, we want to see air in the sinuses. When there is fluid in the sinuses, the sinus cavities look gray, not black. We can see clear ethmoid and maxillary sinuses on both sides. This particular view shows a widely patent ostium of your right maxillary sinus. This should not cause a runny nose.

Some people experience a runny nose after rhinoplasty surgery, and it is not well understood. Symptoms are best treated with topical antihistamine and nasal steroid sprays and saline nasal irrigation. Thank you for this interesting question, and I hope it's helpful. Sincerely, Dr. Joseph
