The alarplasty changed the angle of the nose and made the nostrils smaller. A rhinoplasty can be done with a cartilage graft to increase the tip projection. Also, you need the medial crura pulled together to give you a larger nasal passage.

Hello and thank you for your question. Based on your photographs, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. This can be designed to lessen the pinched appearance of your alar base and can also help improve your breathing. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
