I’ve had alarplasty done 10 yrs ago. I have tiny nostrils. Will a rhino make my nostrils even smaller? I can’t breathe very well, though manageable. I have heard that a rhino may narrow my nostrils even more, but it may help with my breathing. Am not sure if I will end up looking extremely weird with even smaller or narrower nostrils. Also, I have weird lumps on the sides of my alars when I smile. Could that have been caused by my alarplasty? Never had those before the alarplasty.

Answer: Functional rhinoplasty surgery may be considered for nasal obstruction.
Your nose has a very wide base with poor projection. You would benefit from a rhinoplasty. You will need cartilage addition to your septum to increase your projection and make it more triangular. You will need the nostrils narrowed internally as well.

If after surgery a residual hump remains we can rasp it as well as you mention (when its bone), but when we use cartilage grafts in some patients this may cause persistent edema/inflammation

People tend to experience their best breathing, 3 months after rhinoplasty surgery. It is not clear to me why things are worse since that time. Please consider meeting with a reputable outer oncologist for a second opinion about your condition. Thank you. Sincerely, Dr Joseph
