What happens if I just get liposuction done to my lower back? (photo)

I would like to make my butt look bigger and waist smaller so I'm getting my lower back done. the doctor thinks I should do flanks too but I'd like to keep my hips. will it look weird if I just do my lower back?

It's difficult to tell from your picture. I recommend getting a formal evaluation with a hi definition physique expert to go over your desired results. For best results, I recommend a hi definition procedure like VASER liposuction. It's hard to say from just a side view but I don't think your waist size will change much with liposuction. The goal should not just be bigger buttocks but some combination of size and shape. The shape is not visible from just a side view and is a result of the buttock fat distribution and the frame around it. In my experience fat removal from the area just above the iliac crests on the back and over the sacrum contribute most to the appearance of larger buttocks. Doing so does not however affect the shape or the frame. To do that you need fat grafting individualized to your baseline shape and frame. If you just remove love handles fat it does little or nothing to making the buttocks appear larger. The hips lie below and to the sides of the back iliac crests and so are not reduced by the liposuction if anything they would look larger as the area above them is circumferentially narrower.

To get a smaller waist, liposuction is usually done of the lower back AND the sides (flanks). This gives you a smaller waist from the front view as well as the side view. The hips are lower than the flanks, so buy thinning the flanks, the hips will actually look more accentuated and more "curvy." You can ask the doctor to show you the difference on your body if he only does the back vs both the back and the hips. It will give you a more accurate idea of your potential results. The lower back and flank bled into each other around the side and should be addressed at the same time to prevent unnatural stepoffs and contour of the flank area.  Make sure to discuss in full with a board certified surgeon

A nice curve can be created at the back and a beautiful hour glass figure can be achieved by sculpting your abdomen. The fat that is taken out can be grafted at your buttocks at the same time, that will enhance them. I advice you to visit a board certified plastic surgeon and get an assessment done.
