I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

There are 2 possibilities. One is that your implants are bottoming out since they are under the muscle and extruding inferiorly and laterally. The other is the lift failed. This cannot be repaired without surgical intervention.

Your result looks very nice, naturally symmetric, scars obscure, no visible evidence of contracture, and beautiful. You are young and early in your life cycle of breast changes that happen with age, pregnancy, weight fluctuations, etc. Wear good support, especially when physically active. If you want secondary breast lift at some future time to shift breast volume upward, delay until you must have it because every procedure carries new risks.

I would return to your original surgeon and discuss with them. Unfortunately, there are no non-surgical fixes, this will require surgery to fix

To be certain, an exam is needed. However, based on your one photo, I suspect your own tissue is sagging and could be lifted up back into position and remove some excess skin. your implants are probably ok.

To make an accurate assessment regarding the outcome of any plastic surgery procedure we generally need to see a complete set of proper before and after pictures.
If you don’t have before, and after pictures and ask your surgeon to forward, the pictures they took care of based on your picture it looks like your skin and breast tissue has stretched out a bit and this is not uncommon after the operation you had done.
Most likely the skin envelope needs to be re-tightened a bit.
Patient were prone to developing breast ptosis may continue developing some stretching of the skin envelope after a breast lift with or without implants. I suggest you follow up with your plastic surgeon and have him or her address your concerns.
If your surgeon is not available, then schedule a few in person consultations with other plastic surgeons in your community.

I think your soft tissue envelope has stretched. It could be a bit of both.
