Would breast fat transfer's increased volume make the breasts more likely to sag after pregnancy and breastfeeding? Or would the fat transfer give it more structure and not make the breasts sag?

Unless your sister is an identical twin, she cannot donate fat to you. You probably need implants to achieve the size and stability you need. Fat will not give you 2 cups long term, but one half to one.
If you have a living identical twin, fat transfer is possible. Otherwise, the fat would be rejected.
