I was wondering if there were any health risks involved with a bottomed-out implant? I actually like the look of that one more than the one that settled in correctly, as it looks a lot more natural. My only gripe is that one of my nipples is so much higher than the other because of it. I don’t wanna go through another surgery, as I’ve already been through three (not all cosmetic).

These are good medical practice and it depends on surgery planned specially any capsulotomy or moving the implants more medially to give better leakage.
I do agree with going smaller since what you have is too large for your tissue and frame. If after surgery you still have some excess skin, minimally invasive treatment with radiofrequency, like ThermiTight, could be done to tighten your skin.
You did not give any measurements, but based on your photos, 500 cc's will be too large long term. I would suggest not going over 450 cc's, and 400 cc's might even look best long term.
