Volume/fat loss from RF - will a fat transfer to mid face be a good fix? (Photos)

Question

The first photo is before and second is after morpheus. Volume loss under eye and midface causing my cheeks to look heavier and my smile has changed. Im booked in for a consultation with a surgeon about getting a fat transfer and skin pinch under eye but I'm not sure if a fat transfer mid face will be a waste of money or should I look at sculptra

If you truly had volume loss under the eyes and midface after RF (like Morpheus), then fat transfer can be a very reasonable fix, and it’s not automatically a waste of money. The key is choosing the right tool for your problem: lost volume vs lax skin vs swelling/scar changes.
Fat Transfer: When It’s a Good Choice
Fat transfer tends to be best when:
You look hollow under the eyes / flatter through the cheeks
Your goal is natural, long-term volume restoration
You’re okay with the reality that some fat reabsorbs, and you may need a touch-up
It can also improve skin quality in some patients, but it’s primarily a volume solution.
Sculptra: When It May Make More Sense
Sculptra can be helpful when:
You want gradual collagen thickening rather than instant volume
The issue is more soft tissue thinning than a true “dip”
You prefer a non-surgical approach (but it’s still not permanent and takes multiple sessions)
However, Sculptra is not ideal directly under the lower eyelids, and it’s less predictable for precise tear trough correction.
Under-Eye “Skin Pinch”
A skin pinch can help loose crepey skin, but it won’t fix hollowness by itself. Many patients need support/volume first, then conservative skin tightening if appropriate.
Best Next Step
At your consultation, ask the surgeon to clarify:
Is this primarily volume loss, skin laxity, or both?
Would you recommend micro-fat/nano-fat for the under-eye area?
How many fat transfer sessions are commonly needed for a stable result?
