Hollow under eyes - Fat grafting / transfer with or without lower bleph? (Photo)

Question

I want to address the hollows under my eyes. I'm curious whether I need just fat grafting / transfer or if I also likely need a lower bleph (with fat grafting / transfer)? I know this cannot be accurately determined solely by this picture, but I'm interested in thoughts while I await my surgical consults (which have a long wait time). Thanks!

I think it is possible to have good results with either fat grafting vs lower blepharoplasty with fat transfer depending on your surgeon's preference and experience. However, one thing I would certainly consider before moving forward with surgery would be workup for possible thyroid eye disease. This can often lead to issues with the eyes themselves and the lower eyelids, in particular swelling/edema/fluid/discoloration. I would make sure you have this worked up fully by your primary care provider or at least discuss with your surgeon.
