My surgeon has agreed revision, it was agreed the nipple would be lowered to match the other side however he now wants to recut into incision of breast crease and stretch the skin over to move the nipple into a more central position. My questions are will this not likely cause more stretch marks? And also with regards the height difference how will this be lowered to match the lower nipple? Is not best moving the nipple itself instead? If anyone could provide me with advice I would appreciate it

What I think your surgeon is intending to do is to take a wedge of skin out of the lower pole of your breast with the higher nipple to reduce the nipple to fold distance and so lower the nipple on that side. This is unlikely to cause stretch marks. This may improve things for you, but it does tend to be easier and more reliable to match the height of the higher nipple by raising the nipple on the other side. Your surgeon will know what technique was originally used and so will be in the best position to provide you with advice on this. I would recommend making sure that you are consulting a plastic surgeon with a specialist interest in aesthetic breast surgery.

Answer: Adjusting the inframammary fold Without creating visible scar above nipple that needs lowering, which you will not like, the only other option is to lift lower nipple in a symmetric position. This will need intervention on both breast by means of setting new inframammary folds as well as skin excision in the lower pole. You should consider second opinion consultation with a board certified plastic surgeon with good reputation in breast surgery. Good luck.
