I previously had a lefort 1 advancement to correct an underbite but my cheek and under eye area is still quite flat, making me look ssck, tired, and sad. Would malar implants be enough or should i connsider combbined infraorbitaal malar implants? What about submalar and combined malar submalar implants?

It appears that one of the reasons is malar hypoplasia (small cheek bones). Augmentation of the area with small cheek implants, filler or fat grafts,may be the option to improve surrent situation. In person consultation with a board certified plastic surgeon with expertise in this rea would help...
I'd first caution you that the pain may not be related to that implant since it has been in place for so long and your pain is of newer onset. If you can distinctly localize and pinpoint to the pain to that screw, then you may wish to consult with a facial plastic surgeon to remove the screw....
