What's the right implant technique for large outward nipples? (photo)

Thank you for your photo. Some patients have nipples that point straight forward, some patient’s nipples are more medially placed, and some patients, like yourself, have nipples that are wide set. When performing a breast augmentation, the implant should be centered so that the midpoint of the implant is directly behind the nipple. In patients with wide set nipples, this means that the implants sit further to the outside of the chest. I know of no way, without producing some abnormal scarring and shifting the nipple to an abnormal position on the breast, to medialize the nipples on the breast. Best wishes

It's hard to tell from a photo if your overall shape might benefit from a lift to tighten the skin and center the nipples. A breast augmentation alone won't change outward pointing nipples, but it could still look great! Make an appointment to talk through your goals and decide what would work best!

many women seek improved cleavage. Narrowing the cleavage plane can be done very carefully by a skilled surgeon. Moving the implants too far medially can result in synmastia or unaboob which is very difficult to correct. That being said, the right amount of pocket dissection and implant selection can create excellent cleavage. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

It is hard to completely weigh in given the angle of the picture , but you do seem to have laterized nipples , when augmenting the breast some of the already present irregularities might get exaggerated. I would recommend consulting with a plastic surgeon in person so they can obtain a physical exam and standardized images , they might offer you a breast lift to better position the nipple areolar complex at the same time as the augmentation which will give you more volume and upper pole fullness.

Your breasts are large and low on the chest wall. Augmentation alone will not be successful. I recommend a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

From the submitted photo it appears you would have a better result if you had a lift with the augmentation. This would allow your surgeon to address shape and nipple position. With augmentation alone you will have a larger version of your current look.

You would do best with a lift and implants placed behind the muscle. Otherwise, you will just have larger and still sagging breasts. With a lift, the nipples can be positioned where they should be.
