Is my genioplasty bone position too high? (Photos)

Hi. I had a sliding genioplasty 8mm 3 years ago and I’m suffering from lots of tightness, mostly in the fold area of the chin and lower lip. Very tight, stiff and feels like it has little room for movement. Could this be due to the bone position? Is it too high causing the tissues to be compressed? Or is it more to do with scar tissue / adhesions perhaps? I’m desperately trying to find answers and a solution but nobody seems to know. Thank you

It is most likely that the high position of the chin bone (due to the use of the lag screw fixation technique) has compressed the tissues. While the bone's shape has been changed the natural recess for the overlying soft tissue has similarly been reduced., While some patients can tolerate the combination of increased forward projection and vertical shortening, some patients can not and develop the tightness and lack of mobility in the vestibule/labiomental fold that you are describing. The interesting question is whether the treatment for it should be release and an interpositional fat graft or move the bone downward.
