Deep plane, Dual Plane or High SMAS lift?

Question

From previous procedures i have lost the natural cheekbone zygoma contours i naturaly had. Face is wide an flat. I have been advised the 3 different facelifts above by different surgeons and am confused which is right for me to get the contours back and more of a long V line shape that i had with some fullness at the front apples of my cheeks. I want to avoid fat graft if possible. I had natural shadowing along the cheek bone. Which of the above will enable this desired outcome best please?

The type of facelift is far less important than the surgeon you choose. Every surgeon is going to tell you something different. Don’t choose the procedure. Choose the surgeon you think is best ( without getting ripped off ). Expect to spend 10,000 or more

Thank you for sharing your question. This is a fantastic question and highlights some of the most common concerns that patients considering facelift and facelift revision will ask.

A deep plane facelift is likely the most appropriate facelift to address the concerns of:
- restoring a "v-shape" or more youthful oval appearance
- rejuvenating the midface (malar cheek) and temporal area
- avoiding the need for fat transfer (or autologous fat transplantation)

Most SMAS facelift techniques involve removing a portion of the SMAS tissue, which is actually counter-productive to one of the main goals of facial rejuvenation - restoring facing volume. Deep plane facelift procedures allow for preservation of facial volume by repositioning the SMAS tissues of the midface (jowls, nasolabial folds) to a more youthful location over the malar cheek.

Understandably, most patients want to avoid additional procedures like fat transfer, if possible. Deep plane facelift usually can restore midface volume without fat transfer. Although, fat transfer may still be necessary depending on how much fat or SMAS was removed/damaged with your prior procedures .

For most patients, the best facelift option will be based on your individual degree of aging and desired results. Younger patients (often in their 40's and early 50's) and those that have not had prior surgery may be candidates for less invasive options, like mini facelift or other less aggressive SMAS facelift operations (including mini facelift, SMAS facelift, high lateral SMAS facelift, lateral SMAS facelift). If performed correctly these results are significant and long-lasting.
