I have a long nice nose, but little fat just under the thin skin. When i take accutane pills people think i had a nose job! or when apply retinol cream on it, my nose turns to a perfect thin nose. Is there a permenabt procedure to remove little fat under the skin that causes small bumps under the skin and a fuller nose.

Thank you for the question and photos.  The nose is a delicate structure with some soft tissue and of course cartilage.  Technically there are several things that can be injected to dissolve fat but the chances of creating unwanted changes such as cartilage distortion and/or excessive fat reduction is too great.  I agree with you that your nose is very nice as is.  It may not be worth it to risk complications for such a small change.  Surgical reduction of the cartilage or fat in this area may be more controllable, but even this may not be worth pursuing.All the best,Dr. Remus Repta

Thinning the nasal tip particularly if it has a lot of fat will help give you more definition.this is something that any board-certified plastic surgeon experienced in rhinoplasty should be able to do.
