Your droopiness is significative, sever I'd say, but still you are within margins to be your discretional decision; if you don't get a mastopexy your nipples and your implants are going to be low, as low as the nipples are now; to obtain a beautiful result, droopy but beautiful, your new impants can't be located where current ones are, they should be places centered where nipples are now.

Give it a think, a breast lift, in spite the scars, still seems the best choice, however if you go without the lift you could get it anytime in the future.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

Thank you for your question. I have been in practice for over 30 years and revisionary breast surgery is a large portion of my practice. I have taken care of 1000's of cases similar to yours wanting more upper pole fullness. Please do not do a Donut lift. Donut lifts are only for those people who have small areolar asymmetries and do not lift the breast at all. In your case, it would be a disaster. I have developed with a technique termed "reductive augmentation" which I published in 2017. It involves perform an internal bra/capsulorrhaphy, reaugmentation with a larger ultra high-profile implant, removal of inferior pole breast tissue, and revision of your mastopexy/lift. Based on your nicely, post of photos, only increasing the size of the implant with increase the upper pole and continue to give the SAG effect or a double bubble effect. My recommendation would be two stage surgery stage. One would be a full anchor lift breast lift mastopexy waiting 3 to 6 months then insert a 800-1,000cc implant to take a look your appearance you’re looking for expect fees to range from $10,000-$18,000 for both surgeries. A good result depends on how you perceive your breasts. What does a good result look like? If you want a youthful perky breast, then a lift is essential. Size does not determine beauty. And your case a lift alone with implant revision would be more attractive.
