Hello! My upper eyelids keep getting more saggy and wrinkled - especially from age 40 to now 45. The surgeon in my area (has not seen me) typically suggests the upper bleph along with eyebrow lift but in all my research of before and afters (everywhere, not just his work) the brows don't look all that different to me. It's usually the change in the eyelid that I see. I already have a lot of space between my upper lashes and brows so not sure I really need a lift. Are you able to look at my photos and give your opinion? Some are to show lids and some for brow position. Thank you so much!

Answer: Brow lift versus upper Blepharoplasty or both I generally feel a brow lift gives him more natural better Aesthetic outcome, but sometimes the results are only semi permanent as the brow has a tendency to slowly drift down with time. In that sense, an upper eyelid surgery has better longevity. Usually, a combination of both gives the most natural and impressive results. My personal approach would be to do the brow lift first followed by eyelid surgery six or 12 months later. Sometimes people can’t close their eyelids after surgery if both are done at the same time. Ask your provider to open up their portfolio and show you as many before and after pictures as possible, especially of the combination procedure. Bring pictures of your face to use as reference. Ask them to show you as many pictures as possible of previous patients that had similar facial characteristics. An experience used provider should have no shortage of before and after pictures and this is your best way of knowing what your results are likely to look like as well as vetting your provider in regards to experience. You might find the different providers have different opinions for this topic. There are also different approaches on how to do the brow lift in each of these has advantages and disadvantages as well. If you have a high hairline, then you may want to avoid a brow lift that raises your hairline further. Best, Mats Hagstrom MD
