Yes, you need an extended abdominoplasty or a near-circumferential lower body lift. A fleur-de-lis is definitely an option and will remove skin side-to-side in addition to top-to-bottom. However, it comes with a long midline scar. In my own practice, I only use a fleur-de-lis approach in patients who already have a long midline scar (which is unusual). Also, there can be more wound healing issues with this additional scar. My advice would be to look at photos of both procedures.
