Periareolar vs. Double Incision? (Photos)

I prefer minimal scarring but I'm unsure if I have too much skin/tissue for peri. I'm 21, 172cm & 63kg with an 87cm bust, 81.5cm underbust, 18.5cm from sternal notch to nipple on both sides, 5cm from inframammary fold to nipple on the left / 6cm on the right

Since you are young and your over-all breast volume is not too large, a periareolar, subcutaneous mastectomy is the best option. The scar would only be located on the lower half of the areola borders and your results will be pleasing.

Thank you for your question and photos. Based on these photos, I would do an intra-areolar incision which would not leave any visible scars. I treat this like gynecomastia surgery with VASER liposuction, gland excision via an intra-areolar incision and a small (1cm) armpit incision, define the pectoralis major muscle and remove fat from the lateral chest wall. Based on an in person physical examination, Renuvion skin tightening might be recommended at the same time. See a board certified plastic surgeon experienced in advanced body sculpting techniques for an in person consultation/evaluation. Good luck. BTW, the areola will shrink by itself, will not need to be reduced surgically.
