16 years ago I had implants. They started to travel to my armpit and you could see my sternum and a hole or dip near bottom corner. I had a breast lift with implant replacement. My surgeon opened me up and spent 5 hours trying to fix a mess. She said the previous surgeon had cut my pectoral muscle completely in half. Almost 1yr post op. Can I ever workout chest again? How do I prevent the repaired muscle from tearing or separating again? How best to keep them intact. Sleeping in a bra etc?

Risks are low as long as the implant does not continue to drop and thin your tissu where the implant could become exposed.. Your higher side could be treated with ultrasound to see if the pocket could stretch and let it lower too.
These are good medical practice and it depends on surgery planned specially any capsulotomy or moving the implants more medially to give better leakage.
I do agree with going smaller since what you have is too large for your tissue and frame. If after surgery you still have some excess skin, minimally invasive treatment with radiofrequency, like ThermiTight, could be done to tighten your skin.
