Hi there! I’m 36 and over time I’ve gotten more pronounced fat pockets under my eyes when I smile. It makes me look super tired and old. When I’m not smiling that area is really dark. Is there anyway to fix the double roll, fat pocket? I can’t stand it.

Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat creating "eye bags." Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. An in-person exam would provide a more accurate diagnosis. Neurotoxin and lower eyelid surgery are you best options. Part of the “roll” you are seeing is the orbicularis oculi muscle. This can be weakened with neurotoxin to make the “roll” less pronounced. Any fat in the lower lids needs to be addressed with surgery. Best to see an eyelid expert in your area.
