Neurotoxin and lower eyelid surgery are you best options. Part of the “roll” you are seeing is the orbicularis oculi muscle. This can be weakened with neurotoxin to make the “roll” less pronounced. Any fat in the lower lids needs to be addressed with surgery. Best to see an eyelid expert in your area.
