Hello. I had a 390 saline implants placed 3 years ago. My left side sits higher than my right. My doctor has suggested a capsulotomy without removing or replacing the implant. The breast itself is not super firm, mostly it just sits higher than the right. He does not feel it’s capsular contracture upon in person examination. Would this procedure help the implant drop? Also would it be in a lower placement right after surgery or will it be something that takes a moment to drop as I have a vacation in July. Thank you.

This is a classic "double bubble" deformity. This happens when the previous inframmary fold is displaced on to the new lower pole of the breast as the implant descends/settles. This is commonly seen when very large implants are placed, tuberous breast deformity, poor technique/implant selection....

I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time...
