Choosing the correct procedure should be based on having a clear understanding of what the underlying problem is. For your first procedure the problem is that your breast were too small. Now the problem is that your breast have ptosis or droopiness. the treatment for droopy breast is a breast lift. This procedure a.k.a. mastopexy will leave some scars, and that is the inherent drawback. You should probably have your implants replaced at the same time and be a evaluated to make sure you don’t have a capsular contractions, in which case a capsulectomy is indicated as well.
The assessment and treatment options are fairly straightforward. There’s always some difference in skill and approach among plastic surgeons, but breast surgery is fairly common and most plastic surgeons should be able to do a competent job for you.
