Your implant pockets have opened up too much, creating a shadow that represents your old inframammary fold. You can get that fixed with a surgery that re-tightens the pockets, called a capsulorrhaphy. Sometimes a resorbable mesh is also placed for added strength.

That appears to be a slightly under-released pectoralis major muscle inferior attachment. I would go see your original surgeon to voice your findings.

Thanks for your question. I can't see any rippling in the photos provided. Overall, it appears you have a good result. Your right breast may have settled a little lower than the left, but your nipple height still appears to be uniform. I suggest you make an appointment with your plastic surgeon to discuss your concerns.

Your right implant is bottoming out some and the pocket is not as round as before because of this drop. do wear good supporting bras all the time except when sleeping. If need, go back to your surgeon and discuss surgical repair.

From the photos it doesn't appear that the breasts are sagging. I encourage you to return to your surgeon for an evaluation. It doesn't appear to be anything concerning such as capsular contracture, but your surgeon should be able to give you further information after an exam.
