Hi! I am a male in my mid 20s and I am hoping to achieve a more angular/masculine jaw and cheeks - and an overall less rounded and more structured/definited face. I carry excess buccal fat in my cheeks despite being in great shape. This is genetic as my dad has the same thing. I am looking to move forward quickly and find an experienced surgeon to work with. Can also travel for surgery. I have included photos below. What surgeries should I consider?

Answer: Buccal Fat Pad and Chin Lipo Suggestion When a patient has a full and round face and despite weight loss the fullness persist or if the patient is in his normal weight and the chipmunk faces has stayed all the time, a bichectomy or buccal fat pad removal is useful. It´s true that with aging the buccal fat pad tends to diminish in volume do to atrophy, but if the volume is excessive a bichectomy will not hollow the cheek overtime. Many patients are happy with the results, a proper selection is a very important manner. Usually the procedure takes less than 10 min and you can go homethe same day. You'll be able to go back to work on the 3rd day post-op but 1 week off would be more comfort. You can combine the procedure with chin liposuction to get a more masculine, defined chiseled face. So only an in person discussion can figure this out but a brief look at your pictures shows slightly flat cheek bones, faint fat under the chin and rounded cheeks. I would agree that a buccal lipectomy and some jaw chiseling would help but it depends on the look you are going for. If you want the high cheek bone male model look then this is your go to. Though you can do some iplants for cheeks and jaws I would suggest starting with filler and seeing if you like it. The buccal lipectomy will help but not be drastic ie give you a sucked in Zoolander look but rather just enhance the contour of your lower zygoma (cheekbone). Buccal lipectomy is a surgery and can be done in 30 minutes under local anesthesia for $1500. The rest is filler which can be done awake also. Hope that helps!
