Hello, I’m interested in a non-surgical liquid nose job. I have heard some concerning stories about dermal filler procedures. It looks like there are no doctors in San Diego that want to use a doppler-guided ultrasound for this treatment. What’s the reason? It prevents complications from intra-arterial injection of dermal fillers

In our practice, we have been using microdroplet Silikon-1000 for well over 20 years for permanent correction of rhinoplasty irregularities seen through thin nasal skin. There is no risk of necrosis, embolism, blindness or stroke with Silikon-1000 since the size of a Silikon-1000 molecule is smaller than a red blood cell. It’s an oil, not a gel. Candidacy for permanent non-surgical nose job treatments with microdroplet Silikon-1000 may require an examination. Hope this helps! Dr Joseph

Let's start with dispelling the misinformation that doppler ultrasound "prevents complications from intra-arterial injection of dermal fillers."  It can help to visual the arterial anatomy, which may help avoid an intra-arterial injection.  It is not a failsafe prevention.  The devices to perform the ultrasound are expensive, take some skill to apply and interpret, and add time to the procedure.  In the face of the very low incidence of an intra-arterial injection being very, very rare these extra expenses are difficult to justify.  Also, most of the "concerning stories" involve compression on the arteries by the filler and not intra-arterial injection of material.  The fact remains that EVERYTHING that we do, even prescribing a Tylenol, has risk.  Too often in plastic surgery is that axiom forgotten.
