Hello and thank you for your question and the picture. Unfortunately, there aren't any good non-surgical options which would properly achieve what you're looking for! Threads are NOT a good option for the nose, especially for what you're asking (i.e. lifting the nostrils). It sounds like you want to see more of the columella from the side profile view- I'd actually say your columella retracted too far up during either your prior surgery or the healing/scarring process afterwards. To improve the balance and make your nose appear more natural, I'd suggest bringing your columella down and reinforcing it to resist the distorting contractile forces of scarring. A revision rhinoplasty is the only way to achieve this- and it can best address any of your other aesthetic concerns. I would be able to adjust the size, shape, and definition of your nose (including the nostrils, columella, and tip)! That said, revision rhinoplasty is truly a surgery of millimeters- they are the most technically challenging procedures in all of plastic surgery! Because your nose is the DEFINING feature of your face- I would only recommend seeing a surgeon who specializes in revision rhinoplasty procedures, and who has significant training and experience performing these surgeries. I actually trained under one of the world leading revision rhinoplasty experts (who performs 1,000+ nosejobs annually).
