Under-Eye Bags from fillers are typically result of lymphedema (lymphatic obstruction). These can be difficult to treat. However, I have had gratifying results using varying combinations, concentrations, and amounts of intralesional anti-inflammatory agents. Based on these photos, bilateral ptosis correction with lateral canthopexy would be procedures to start with. More elaborate consultation needs to be done in person with a board certified plastic or Oculoplastic surgeon. Good luck.
I recommend a brow lift at the frontal hairline. This will decrease the vertical height of the forehead, lower the hairline and pull up the eyebrows. I do not believe you need a blepharoplasty at this time. Best Wishes, Gary Horndeski, M.D.
