Stage 3 inverted nipples and tubular breasts, what are my options to get lifted perky ones? (photos)

I am 23 years old. I have done some research and believe I have stage 3 inverted nipples and tubular breasts. I do not have muscle behind my breasts, it just feels soft and saggy. I do not feel sensation on the nipple area/they don't erect. Is there any supplements I could take to reduce the puffiness of the areola? I am wondering what kind of pressures I'd have to go through to get lifted perky breasts and also the cost of it. Does anyone recommended Surgeons in Tijuana, Mexico.

Hi Delabube,Thanks for your photos and questions. The results will be based on what you realistically expect. You may be able to avoid a lift but the nipple areola complex will stretch and likely not correct the nipples. Based on the photos, your nipples dont seem totally inverted but without an exam it is hard to say how to fix the issue. A round implant over or under the muscle with more breast tissue coverage than muscle (Dual plane II/III) with a donut lift (around the nipple) lift would look well and give what I think is what you want to see. As far as surgeons, Tijuana will be a lot less but you carry a higher risk. There are good surgeons everywhere but remember you only get one lift and is the cost the most important aspect of your future surgery. Only you can answer this. you are a good candidate for the lollipop type lift and a small implant.  That would give you nice shaped breasts.  As far as inverted nipples, those may also improve from the pressure of an implant behind them.  I would not go down to Mexico for the simple reason that you may need a touch up or revision and going back to Mexico would be quite inconvenient and add to the cost.

Constricted or tubular breasts tend to be too narrow with a breast crease that is too high. Tight ring-like bands of tissue around the areolae and nipples may create the appearance of protruding or puffy areolae. A higher than normal breast crease may make your nipples appear to be too low or down pointing. Constricted breast features can be very mild or severe. In fact, mild variations are relatively common. To summarize, the features can include:

Inframammary fold (breast crease) too highInframammary fold misshapenBreasts too narrow (disproportionately narrow base width)Insufficient breast skinConstricted or tubular shape, resulting from tight bands of tissue within breastsProtruding or bulging appearance of areolae (pigmented area around nipples)Improvement is possible and generally includes a breast augmentation, sometimes combined with a lift (mastopexy). The lift is usually limited to an incision around the areolae. During the procedure, I release the tight, constricting bands from the inside, allowing the skin to expand properly. I also lower the inframammary fold to a more ideal position and improve it’s shape. Breast implants help create the right amount of volume and improve breast contours, resulting in a more rounded or tear drop shape. In some cases I have been able to fix this with just fat grafting in milder cases.

Thank you for your photos. Your breasts have tuberous characteristics including lower nipple position, narrow breast width, and puffy/enlarged areolas. You may benefit from both a breast lift and implant placement with special techniques used to score and expand your breast tissue to change it from a cylinder (tube) to a more conical and natural shape. At the same time, the areolas can be reduced in size and made more symmetric using the same incisions used for the lift and implants. Sometimes, the nipples will have improved projection after a lift and implants without any direct manipulations or incisions which carry the risk of reduced sensation or difficulty breastfeeding. If needed, a local procedure can be done to evert the nipple at a later time. It is important to have a thorough in-office examination so the right technique and implants can be determined. Based of your pictures you will need a breast lift with breast augmentation for more volume.What are the basic tips to know if I need a breast augmentation or breast lift? It all depends on the position of the nipple on the breast. The further down the nipple is in relation to the fold the more the chances are the patient needs a lift.   High profile are taller and narrower implants when compared to moderate profile implants of the same volume.The decision is usually based on the patient breast dimensions, amount of breast that the patient can contribute to the projection and I also factor the patients desires. I try my best to also respect the skin quality and elasticity in order to prevent future drooping due to unnecessarily heavy implantsover or under muscle doesn't dictate what size implant we can use.  The muscle coverage is helpful in very thin patients and seems to reduce capsular contracture according to some scientific studies. Under the breast tissue is the most natural feel and has a less chance of animation deformity when the pectoralis muscle contracts. In most instances only the top implant is covered by muscle and the reset of the implant ends up under the breast tissue.Choose a board certified plastic surgeon who has done a ton of breast augmentation  and has privileges to do this surgery at a local university hospital. This says his credentials have been checked out by fellow Drs.
