Interesting question. Requests come in more often for the opposite as certain ethnicities (Asian females) can be victim to "cankles" where they do not like how their calves taper too subtley to their ankles. In short, yes, fat grafting or placing a custom implant would work. Custom implants would likely leave a scar, though. Fat grafting has an advantage of being permanent without leaving scars. Post operatively, You would have to keep your legs elevated without any compression to the area for several weeks - this is something to consider. Another non-surgical alternative would be Botox for your calves to shrink down the muscle there so that it may be in better proportion with your ankles. There are lifestyle adjustments to this, however. Best to see a board certified plastic surgeon in person to discuss your goals.
