I lost 20 pounds and my breasts are deflated up top. I’ve never been satisfied with my breasts as they sag quite a bit, even when I was chunkier the sag was more obvious. Looking at these pics do you recommend a lift with BA or only a BA? I would prefer a lift w/BA if the BA alone would sag my breasts more at the bottom even with upper fullness, but I’m afraid that since my nipple position is in a good place that the lift would leave it too high up. What can be done about this?

Answer: Do I need a #breastlift or will #breastimplants alone do the job? Thanks for the question and photos. You are one of those patients on the border who have what we call glandular ptosis. Your breast volume is bottom heavy but your nipples have not yet rotated down or descended below the folds. Patients with this characteristic can often get a great result with breast augmentation alone, but they need to understand there will be some hang/bounce/lilt (whatever you want to call it), with the implant adding weight to the breast and potentially having the volume concentrated low on the chest wall. Over time, this may further stretch your lower breast skin envelope and lead to true sagging of the implants and native breast tissue. A small lift to provide skin envelope support and move the nipple/areola complexes a bit higher would be appropriate, but is not absolutely necessary from what I see in the photos. Obviously, exam in the office is required to fully counsel a patient. We use Crisalix surgical simulation software to help show patients the potential outcomes with and without a lift and with different size/style implants, which I find very helpful in directing patients in their surgical planning.

Having said that you are still a good candidate for implants alone and then wait and see with the lift. I do though ask women in your situation what bothers them more the size and hollow above or is it the sag. If it really is the sag that bothers you the most, then a lift is real consideration.
