Sometimes changing an aspect of one portion of the nose can inadvertently causing another portion of the nose to appear larger or smaller. While some procedures can be performed and under local anesthesia (such as an alarplasty) other procedures (such as grafts or tip work) may require a general anesthetic. If alarplasty is the only procedure indicated, then it can usually be performed in the office under local anesthesia. Please see a surgeon who is certified by the American Board of Plastic Surgery who is experienced in rhinoplasty; for instance, I have been doing this procedure since the mid 1980's.
