The dorsum, or spine of the nose, may be widened if it is too narrow naturally or secondary to surgery. The dorsum may need to be taken down (like taking the top off of a mountain) and this is sometimes enough to give a wider appearance. If not, the nasal bones can be out fractured (pushed out). A spreader graft, cartilage taken from inside the nose, can be placed to hold the width, if necessary.
Nasal bones which are overly wide or narrow can be reshaped either with osteoomies or with grafting. Using spreader grafts, the middle portion of the nose can be augmented at the same time.
