I'm a 26 y.o female and have been contemplating a rhinoplasty for years. I don't want to do anything too drastic I would just like to reduce the tip of my nose. Sometimes I feel like it gets swollen, making my nose look bigger. I have had a horrific experience before on a prior surgery, Because of this just the thought of going under gives me anxiety and makes me want to cry. This is why I want to know if surgery is do-able while awake.
Answer: No- I recommend rhinoplasty surgery Hello and thank you for your question. Unfortunately no, surgery is the best option to address the size and particularly refine the definition of the nasal tip, as in your situation. The less defined appearance of the tip is due in part to the thickness of the skin and also due to the underlying anatomy present- a combination of your nasal bones and cartilage which makes the structural framework of your nose. Addressing the shape of the nose (especially the nasal tip) is actually one of the most common reasons people seek out and undergo a rhinoplasty procedure. In addition to addressing your aesthetic concerns, your septal cartilage (or an off-the-shelf alternative) would be harvested (removed) and turned into multiple cartilage grafts, which would then be secured into different areas of your nose for structural support, including in the columella (middle bottom portion of your nose between your nostrils) to really reinforce the tip and prevent long term alterations/distortions to nose shape. The open approach is very effective in facilitating complex, precise maneuvers to refine the nasal shape and size to a particular aesthetic and look. Rhinoplasty is arguably the most difficult procedure to perform in all of plastic surgery. I would only recommend going to see a surgeon who feels very comfortable with both primary and revision rhinoplasty procedures, and who has significant training and experience in these types of surgeries. This is a surgery of millimeters, so if the surgeon you're going to only does 1-2 per month, you are not going to get the best result possible. Of course it is always difficult to provide tentative surgical recommendations, as there is only so much information to be gained from the photos and description provided. Dr. Donald Groves, our expert facial plastic surgeon, has extensive training and experience with complex rhinoplasty surgeries, so I’d recommend for you to come see him (or another plastic surgeon who is extremely comfortable with rhinoplasties) for a formal consultation to thoroughly discuss your surgical goals, undergo a formal examination, evaluate all your options (surgical and nonsurgical), and decide on the best procedure(s) and type of anesthesia for your specific case. I hope this helps! Dr. Emer

Answer: A permanent nonsurgical nose job may not lead to a narrower nasal tip appearance. Based on your photos, you appear to have a pleasing nasal appearance. In our experience, adding volume to the nasal tip may lead to an even wider nasal tip appearance. I hope this is helpful and thank you for your question. Sincerely, Dr. Joseph

Answer: Non surgical rhinoplasty Hello, A non surgical rhinoplasty is performed by injecting hyaluronic acid filler to ADD volume, the only way to take away volume is with surgery. That being said, filler injected in the right places can change the shape of the nose to give the illusion that it is smaller and fades into the background of the face. I recommend an in-person consultation with an experienced injector to ensure that you are a good candidate for non surgical rhinoplasty. Best of luck!

Answer: Can you have a nasal tip plasty under local anesthesia? Can you have a nasal tip plasty under local anesthesia? Nasal tip plasty can be done under local anesthesia, or local anesthesia with some mild oral sedation. Generally patients are much more comfortable asleep. But if the anxiety of anesthesia is worse than the anxiety of having multiple shots of local anesthesia injected into your nose it is doable.

Answer: Look up #bostontipflip for nose tip flip! While fillers add volume to the nose, the appearance can be smaller if done correctly. Look up #bostontipflip to see many cases where it's been done. My recommendation is try it and see if you like it before signing up for surgery. At the very least you'll be able to show your surgeon the results you'd are looking to get.
